- This is a "referral only" practice . This means I need to be contacted by your primary care provider (PCP) and/or counselor/therapist before I will make an appointment with you. Your PCP must be willing to collaborate in your care, meaning willing to provide "bridge" prescriptions under urgent circumstances, and to provide ongoing refills once you are truly stabilized, as all psychiatrists in Vermont have waiting lists.
- You need to have had a minimal screening physical examination within the past year that your PCP can send me.
- Your counselor/therapist must feel comfortable sharing information about your therapeutic process with me as necessary, and must have his or her own coverage when away.
- You must be willing to sign releases for all prior treators.
- You need to check with your insurance company about any procedures or prior approvals that need to be done, and have that information to give me when you call for your appointment.
- I am happy to discuss with you the reasons for these restrictions when we first meet.
